1. Eligibility. The Premier Instrument Recycling Program (the “Program”) is open to all licensed dental professionals (except those who reside in and/or are licensed in Vermont) who are at least 18 years old and residents of the United States.  Schools and universities are not eligible to participate in the Program.

2. No Purchase Necessary. NO PURCHASE OF ANY KIND IS NECESSARY to participate in the Program.

3. Processing Fee. A processing fee will be charged by Premier Dental Products Company (“Premier”) to facilitate your participation in the Program. Premier may change the processing fee from time to time without additional notice. Premier uses a third-party payment processor to process all payment transactions and may change the third-party processor at any time without additional notice. Premier does not collect, process or store participants’ payment information, such as credit card information. By participating in the Program, the Participant acknowledges and agrees that all participant payment information is collected, processed and stored by the third-party payment processor and is subject to the processor’s posted privacy policy and terms of use. Premier expressly disclaims all responsibility for and will have no liability to a Participant in connection with the services provided by the third-party payment processor. By participating in the Program, each Participant agrees to be bound by the separate terms and conditions applicable to the third-party payment processing services and authorizes Premier to have the payment processor charge the amounts owed to the payment method provided by the Participant.

4. Reporting Obligations. BY PARTICIPATING IN THE PROGRAM, EACH PARTICIPANT AGREES TO COMPLY WITH ALL APPLICABLE SUNSHINE REPORTING OBLIGATIONS IF THEY ARE A COVERED RECIPIENT, AND TO PROVIDE PREMIER DENTAL PRODUCTS COMPANY WITH ANY INFORMATION NECESSARY FOR ITS SUNSHINE LAW REPORTING OBLIGATIONS.

5. Release and Indemnity. By participating in the Program, the Participant fully and completely releases, discharges, indemnifies, and holds harmless Premier and its affiliated companies, directors, officers, employees, representatives and agencies, as well as Facebook or Instagram, from any and all claims, costs, injuries, losses or damages of any kind arising out of or in connection with the Program. Further, Premier is not responsible or liable for any failure of a Participant to return their instruments in accordance with instructions or any stolen, incomplete, lost, or damaged instruments.

6. Terms of Use. Premier’s Terms of Use available at Premier Dental | Terms of Use (premierdentalco.com) (“Premier Terms”) and Premier’s Privacy Policy available at https://www.premierdentalco.com/privacy-policy/ (“Premier Privacy Policy”), are incorporated herein by reference. For the avoidance of doubt, all references to these Terms and Conditions will be interpreted to include the Terms of Use, and all references to “Site” in the Premier Terms of Use will be interpreted to include this webpage.  In the event of any conflict in terms between these Terms and Conditions and those found in the Premier Terms or Premier Privacy Policy, the terms contained herein shall control.

7. Miscellaneous. The Premier Instrument Recycling Program and all accompanying materials are copyright 2024 by Company. PREMIER is a registered trademark of Premier Dental Products Company. Participants agree to be bound by these terms and conditions. Premier Dental Products Company reserves the right to modify these terms and conditions or any aspect of the Program or cancel the Premier Instrument Recycling Program at any time at its sole discretion. These changes will be effective immediately without additional notice.
